New patients have a number of forms to fill out prior to seeing the dentist or hygienist. The first form to fill out is the release of records. Please contact us to complete all necessary forms.
Our staff will be happy to sit with you in our office to complete the forms together. We’re all about making your visit smooth and hassle-free. To ensure we maximize your appointment, please remember to bring your insurance card, if you have one; medical and dental history forms; and any questions or concerns you may have.
